Collaborative Care

Behavioral Healthcare

The Collaborative Care Program (CoCM) is a team approach to the treatment of anxiety and depression in primary care. The goal of CoCM is to increase access and reduce the stigma associated with accessing treatment for mental health.

The team includes you, your doctor, the behavioral healthcare manager, and a psychiatric consultant.

Prospective patients can request that their doctor refer them to receive CoCM services. After a referral, patients will meet with their Behavioral Healthcare Manager to create a care plan that best fits them.

Jill Levine MSW, LICSW practices within an integrative health care model for a private primary care practice whose strategy is to closely coordinate mental health counseling efforts and social service delivery with patients and their physicians who are at the center of this initiative.

Jill Levine began practicing over 25 years ago servicing individuals and families while incorporating a variety of therapeutic modalities using primarily a strengths based approach.

Her years of experience encompasses working with both the private and public sectors, including: state, social service, home care delivery, community health agencies, private consulting to the Courts, and being an approved instructor for mandated parenting classes. This diverse background allows Jill to operate out of a generalist approach understanding how individuals and families needs change through their developmental lifecycle.

She looks forward to enhancing lives while delivering a collaborative problem solving goal oriented experience with her clients. This approach enables Jill to proactively connect with her clients, empowering them toward managing stressors while incorporating appropriate resources to establish and promote positive outcomes.

Dr. Medlock graduated from the University of Notre Dame followed by the University of Florida College of Medicine.

He completed neurosurgical training at the University of Illinois and fellowship training at Boston Children's Hospital.

He spent 5 years on the faculty of Georgetown University in Washington DC before returning to the Harvard Medical School, Massachusetts General Hospital, and North Shore Medical Center.

He retired from operative neurological surgery in 2017 and now practices a combination of non-operative neurological surgery and addiction medicine.

While he always enjoyed the demanding work under the operating microscope with the brain and spinal cord, he knew there were other brain diseases that were having an enormous impact on our public health: addiction and substance use disorders.

Dr. Medlock is enjoying being a part of the solution to these troublesome yet treatable diseases.
